Dean is an omega, Sam and Castiel are both alphas, and their plan when they got together was for Castiel alone to fuck Dean during heats (so that there would be no incest-born babies). But one morning Dean wakes up feeling only a little hornier than usual, and Sam fucks him without realising he's going into heat. The main body of the heat never happens, and a little while later the three of them are forced to accept that Dean is in fact pregnant with Sam's child.

What happens next is up to you, though I'd like Dean to carry the baby to term (maybe with Castiel keeping an eye on it to make sure there are no side effects of its parentage?)

Inspired by Dean's comments when he settles into the Batcave: he really is nesting. Like, making a den of sorts and filling it with duvets and pillows and soft things ready to mate. He's never been particularly fertile before- he has to feel safe and loved before he can be- but now that he is he can feel his first-ever heat coming on, and he wants to be ready for it.

Dean himself didn't know why he was doing it. He'd been feeling odd lately, ever since Castiel had come back. They'd had a quickie, his first ever sex with an angel, and Cas had done something weird to him and since then he'd been looking for a place to settle down. This was grand, this was perfect and now he was compelled to fill his room with soft, plush things. At Sam's request he moved all the pillows and cushions out of the main room and just into his own bedroom. It was difficult to walk through now, but he felt much better. Dean could curl up pretty much anywhere, and even though he normally spread out like a lazy starfish, he slept better now
