And so they finally give in to it all and get off together in some way, and the next morning Sam is acting like it never happened; now that it's done, he can't tell whether it was real or imagined and is erring on the side of caution (which he tells himself, duh, is "didn't actually fool around with my big brother last night," because the Stone #1 of "Dean is always real" doesn't hold up to incest in the cold hard light of day).

So now Dean is worrying that Sam didn't really know what was going on at the time, and that it wasn't quite the life-affirming-funtimes-maybe-it'll-all-be-okay-now thing he thought it was. Because Dean needs something else about which to feel guilty :).
