Sam has always been certain he'll end up an alpha, just like his dad and brother -- but he's also a little terrified of ending up an omega. He's certain that his kind of macho hunter family will reject him if he is.

They're in a bar, John and Dean hustling pool, when Sam(no younger than 15, please) suddenly goes into his first heat.

All the other alphas in the bar start sniffing around him, and he's terrified. To protect Sam, John claims him over the bar in front of all the other alphas(public mating isn't really seen as too odd) -- John being as gentle as he can be, trying to look after Sam. Dean protects his family, keeping the other alphas back while the mating takes place. John ends up wrapping Sam in his coat and carrying him out to the car, afterwards.

I would totally love, as bonus, an extra schmoopy end, where they all talk about what this means, and how Sam has nothing to worry about -- that John and Dean are always going to look out for him and take care of him.

Some kind of world set up where the upper class can buy bedmates/servants, and it's all legal/normal.

Jensen's not a bad guy. It's not like he thinks that they're not people. They're just not people people.
