Age: Unknown. If it is assumed that Medicine is a doll
Tsukumogami, though, then her body must be at least 100 years old.
As for how long she's been 'alive', her canon profile in the first game she was in said that only a few years had passed since she became a youkai. It can also be assumed that she's been properly alive for less than sixty years, as in Phantasmagoria of Flower View (TH09) she did not recognise that the events that were happening came around one time sixty years ago. I'm going to guess (and headcanon) that she was five years old in PoFV, and is ten in the present-day.
~*Magical*~ abilities and strengths: Like most other Touhou characters, Medicine has the ability to fly at will, and to create hundreds of little projectile bullets in varying patterns in what is known as 'danmaku', or curtain fire. Danmaku is a non-lethal way for youkai and humans to settle disputes on an equal level, sometimes using more tricky or specialized bullet patterns in 'spell cards'. Generally, danmaku fights are won by your ability to dodge bullets and use bombs at the right time (in-game, bombs usually clear some bullets away), and not your power. This is why even very, very powerful youkai, such as Yukari Yakumo and Yuuka Kazami, can sometimes be defeated in danmaku battles by the protagonists.
What is unique about Medicine, however, is that she can use and manipulate poison at will. The type of poison she uses could be any number of poisons and venoms, which makes it very dangerous, as there may not be a specific way to treat the poison. In-game, the poison she uses has the effect of slowing down the player, making it more difficult to dodge the many bullets being sent their way. Touching her skin or her clothes also has the effect of inflaming the skin, due to it being coated in poisons, unless you're wearing protective clothing. Incidentally, those with sensitive noses might notice that she smells because of this.
Despite her inexperience and her low intelligence when compared to a human, she's very knowledgeable about poison. In PoFV, she tells some characters that the best poison is one that slowly circulates and deteriorates everything, and that a poison that's too strong will be immediately cleared out.
Another very noteworthy thing about Medicine is that since she's a doll, she doesn't need to eat or sleep to sustain herself. She can if she wants to, but it won't serve any real purpose. She is also unaffected by poison, and won't 'die' unless every last piece of her is completely destroyed.
How would they use their abilities?: Medicine hates humans, and she also doesn't know the meaning of holding back, so it's very likely that she'll just attack someone with poison for no reason unless they manage to convince her otherwise. Luckily, she's pretty easy to fool - just say that you hate humans, even if you are one yourself. Even then, she might still poison you if she doesn't like you. It's very normal in Gensokyo (the setting of the Touhou games) for random danmaku fights to break out, too.
Appearance: Medicine, appropriately, has a very doll-like appearance. Red bows adorn her short, blonde, wavy hair and the frilly white collar of her black shirt, with another big, whitish-pink bow on the back of her red skirt. Red and black dashes are woven into her clothes, forming crosses where they meet, and she wears little red shoes with frilly white socks. Her skin is perfect and pale, and her eyes are a greyish blue. While she's large for a doll, she's small and petite for a youkai - you'd be forgiven for thinking she's a child.
A very small doll with fairy wings follows Medicine wherever she goes. It almost mirrors her appearance completely - it wears a little black dress, with what seems to be a red or halfway-red shirt, and wears a red ribbon in its blonde hair. Nobody ever seems to notice or make reference to this little doll, not even Medicine herself, so its exact purpose is a mystery. Some speculate that this doll is the 'real' Medicine, and it controls the larger doll who we think is Medicine, but canon says nothing about that.

Background/Personality: As with many other Touhou characters, Medicine has had very little screentime. The only info about her personality and background comes from a short profile in the game's manual, her dialogue in the game, and an article about her in an official fanbook. I've tried to stick as close to canon as possible with this, but I may have had to fill in some gaps with headcanon.
A long, long time ago, a very large doll that may have been used in ventriloquism was thrown away on Nameless Hill, a hill surrounded by poisonous Suzuran (Lily of the Valley) flowers. Over a long time, the suzuran gave it the ability to control poison, and brought it to life. This doll was Medicine Melancholy.
It's unknown how she came up with that name, or how she even learned to speak. It's possible that she may have retained the knowledge and that name from the time where she belonged to someone, as humans do tend to pour their feelings into dolls. Nevertheless, she was discarded, and the abandonment made her absolutely despise humans. Being a doll herself, it also made her a very big advocate of doll's rights. The problem being that no other dolls are alive like she is, and therefore no other dolls are able to support her cause.
Humans rarely went to Nameless Hill because of all the poisonous flowers, so for the most part, her only company was the suzuran, or Su-san, as she began to call it. As the nickname may suggest, Medicine is very affectionate and protective towards the flowers. She'll often talk about them and talk to them. In PoFV, Komachi Onozuka asks if the poison has sunk deep into her brain, too. To which Medicine replies, "Of course, I'm being fulfilled". She's perfectly content to admit that the poison may be making her hallucinate sometimes, and she's happy to ramble for ages on end about poison and all its effects.
As a result of living alone with the suzuran for so long, she became rather close-minded, blunt, and perhaps even naive, with a lack of knowledge about the world. Very rarely, she would sneak down to the nearby human village just to look, but having never seen a large number of humans, she would become startled and run back to her field of flowers. From these sightings and the very little the humans knew about Medicine, they worried that she may return to spread a fog of poison over the village if provoked. In the end, they figured the best thing to do would be to not pursue her any further.
Things became a little interesting one year. Spring arrived, and the suzuran were blooming nicely. But, they seemed to be blooming unnaturally much. Many characters passed by the suzuran field, and Medicine actually left, herself, and many danmaku battles were had - everyone was trying to find out the truth behind the abnormal spring. Medicine didn't really have any interest in finding out what was going on, and just used the opportunity to happily try to defeat anyone she came across with poisonous flowers. She was also delighted to find some other poisonous flowers blooming, the Higanbana (or, spider lily).
Along the way she met Shikieiki Yamaxanadu, the judge of the deceased, who lectured Medicine on all her faults, saying that she's too narrow-minded and nobody will want to help her in her cause for doll liberation. Her words were shocking to Medicine, and it made her think. While she wouldn't give up her hate for humans so easily, she decided that maybe she should be a little more friendly to others. More allies would mean more support for doll rights, after all. She even formed a sort-of friendship with Eirin Yagokoro, who along with Reisen Udongein Inaba, was the first to pass by the suzuran field after the incident.
Even so, nobody visited the suzuran field much. The next visit she got came from Aya Shameimaru, a crow tengu reporter who she'd met previously during the incident. Medicine was all too happy to let Aya take pictures of all her spellcard patterns. Nobody but Aya knows the purpose of the photos she takes, but Medicine naively believed that it would be the next step in spreading word about the suzuran field.
Time passed, but she heard nothing. Sometimes she left the suzuran field, but more often than not she would bump into a human, and would flee back to the flowers. She tried to be friendly to any youkai who visited her field, but sometimes she'd still end up attacking them with poison anyway. Needless to say, any humans were definitely attacked with poison, although she did try to hear them out sometimes. Shikieiki's advice started to fade from her mind, lost in a fog of poison. But she still tried.
Even after everything, Medicine can be seen as pretty creepy and sinister, thanks to her love of poisons. She once remarked that it felt great to defeat living things, and that feeling only grew as she defeated more people. She's also happy to kill humans if they really, really upset her, and it even makes her smile to watch their life fade away. She's still socially-awkward (if the killing humans thing didn't tip you off) and unintelligent, resorting to simple "I like you"s or "I hate you"s when she can't find the right words for her emotions, but at least there's not a 100% chance of her poisoning you anymore.
The fact remains that it's easy to provoke her into attacking at full strength, making her a danger to humans and youkai alike if they're not skilled with words. It's best to exercise caution when dealing with her.

[The feed is focused on a young looking girl, who appears to be sitting down on the floor in front of her Guide. No tears fall from her glassy eyes, but it's clear from her expression and the sound of her voice that she's very upset.]
Su-san... are you really gone?
[She draws up her knees, hugging them to her chest and hiding her mouth from view. Her next words sound a bit muffled.]
I miss you, Su-san. I hope I can find you. I hope you're not lonely.
[Her eyes close, and she's silent for a while.]
If anyone's hurt you... I'll make them pay, Su-san.
[Her expression hardens, and the sadness in her voice disappears, replaced with a hint of anger.]
That's right. I'll make them pay.
[She picks up the Guide and stares into it, a smile on her face.]
I'll make them pay for what they did to Su-san.
[The feed ends.]
3rd person sample:
Wherever she went, she still couldn't stop thinking about Su-san. It wasn't like Medicine to ever be sad about anything for too long, but... could the suzuran field really be gone? Was Gensokyo really destroyed?
A flash of anger coursed through her. Why would anyone do this? She couldn't even bear to think about it, she just... she hated them, whoever did this. Whoever was horrible enough to destroy Su-san was as bad as a human. No, maybe even worse. She'd kill them... if she ever found them, she'd kill them.
Lost in her hateful thoughts, she ending up walking straight into a man.
She gave a little shriek in surprise, and found herself falling over.
"Oh! I'm sorry," said the man, quickly turning to Medicine with a hand over his mouth. "Here, let me help you up."
Before Medicine could protest, the man had taken her hands and pulled her to her feet. The man probably didn't expect her to be so light, and he probably didn't expect his skin to suddenly get red and itchy where he'd touched her, either.
"Huh? How...?" he gasped.
"Stay away from me." Medicine had backed off a considerable amount the moment she was back on her feet again. "Stay away from me, or I'll poison you."
"Hey, now..." Something about the way she was glaring at him made him feel really nervous. "I was only trying to help you."
"I don't need your help! I don't need a human's help!" she yelled.
"Okay, okay, I'm sorry! Calm down!"
But it was too late - she'd released a fog of poison at him. It took a second, but as soon as he realised what was happening, he gasped and tried to flee as quickly as he could. Medicine watched him run; even from a distance she could see the poison taking effect, as the man's movements became more difficult and sluggish.
Maybe he'd survive.
Maybe he wouldn't. The very thought made her smile.
