Mpreg is so not my thing but I've been craving fic where Castiel has swollen, tender, oversensitized nipples and sometimes mpreg does that so I gave this one a shot. Then there was lactation (I missed the tag!) and had to flee.
If these are your kinks, though, you might like it!

I would rec it. It's a D/C Sound of Music fusion, with nanny!Dean doing a lot of patient explaining to seven half-human/half-angel children, who have been, up to this point, raised by Castiel. When I checked out the author's other stuff, I saw that all of their stories include the OC children, and I skimmed around a bit and wasn't interested. But I like them in this particular story.
