Yes, so much to love about this episode! It took me twice as long to watch because i had to keep going back to rewatch all the pretty. There are, of course, things I still want. For example, definitely could have used a scene of Sam waking up in hospital and Dean's relief that it's really, finally over. Secondly, Dean knows death when he sees it. I thought the shock excuse sounded pretty flimsy and i saw a suggestion somewhere that Sam did actually die but Chuck brought him back because he needs both Winchesters to defeat Amara. Maybe he brought Dean back too.
I had a similar thought, but about Billie. Certainly with Sam basically comatose and still bleeding out, it seems a stretch that he'd wake up and be able to kill two werewolves! Does beg the question.
I thought the exact same thing. Two possible explanations. Someone gave Sam just enough life to succeed without playing their hand. And two, of course, he's Sam Fucking Winchester.
I've seen speculation that it was Lucifer, too, but the problem there I think is that Lucifer still can't find Sam and Dean unless he knows where they are? Sam still has the runes on his ribs, right? Unless Gadreel got rid of them or something? (I didn't watch a lot of S9.)
I like that theory less, simply because Lucifer didn't seem to interested in keeping Sam alive back in 11x14. I have no idea about the angelic runes to be honest. I'm more inclined to think of Chuck because i know he's going to be in an episode shortly. But it could also be Billie who has to talk a big game but knows the stakes are much too high to reap a Winchester right now.

There's that, too, although I haven't really liked they way they're writing Lucifer this season, nor the fact that they left Sam being Lucifer's perfect vessel by the wayside, so I wouldn't mind if they went there. But I'm pretty sure they still have the runes, plus I think you're right that they're letting Lucifer run amok, and he's not particularly concerned with the Winchesters right now. Seems a less likely explanation to me than Chuck or Billie.
*jumps in* I think the idea there was "something else* keeping Sam alive is interesting - and it definitely makes more sense than his body going into shock, but I would be so surprised if anything comes of that later on as I think they would have dropped more of a hint during the episode (maybe?). I think we can certainly interpret that something helped him, but I'm not convinced the show had that in ming.
Someone suggested on my review thread that if Sam was near death (like Dean was) Billie should have appeared to him. As in, she would have known he was very close to death. I suspect it's just part of the narrative and the tension, but I would love to see that be something more in future episodes. :)
Yeah, I wasn't thinking they'd do anything with it, but the question is interesting to think about. And yes, Billie even said she would have known if he was dying, so that kind of makes the Chuck theory even a little more likely-looking to me. Unless it was Billie and she was trying to teach Dean a lesson. (Good luck with that!)
I always put stuff like that down to Sam and Dean being basically "bred" to be vessels for Archangels. Lucifer's temporary vessel was looking more like a corpse than a man by the time Sam said yes. In "The End", however, Lucifer had been in Sam for years and he looked better than ever. On top of which, Sam's got the demon blood.
They are definitely significantly more resilient than the average human. Part of it too though is that Sam definitely had blood on his lips when he went down which suggests internal injuries to me. Now he also could have just been punched in the face but i still think something beyond him was keeping him alive.

I'm more inclined to think of Chuck because i know he's going to be in an episode shortly.
But it could also be Billie who has to talk a big game but knows the stakes are much too high to reap a Winchester right now.
