Isolated. Never pandemic. Usually, something else is involved -- usually magic or aliens, such as the Black Lanterns.

So they were controlled. And created by unnatural powers. [Thinking out loud for a second] Aliens ... someone would have said something about that.

Did they spread their symptoms to others? The usual "you get bitten, you turn into one of them"?

Not necessarily. Created, yes, controlled... it depends.

In some cases, such as the Black Lanterns. Though it was more... uh, killing and affecting via the rings that did it. I believe some suffered minor changes and hallucinations when bitten, but once the bonds were broken they returned to the way they had been prior.

If it was magic that caused it and if the magic that caused it wasn't from high-tier Hellspawn. Demons, particularly Demon Lords usually have to be coaxed to remove a spell themselves.

As for aliens -- such as the Black Lanterns -- they responded to emotions. All you could do was fight them and attempt to remove the links that they had with the living (or formerly living) host bodies via "light," if you will. Though it's hard to describe what they were, considering Nekron wasn't exactly... an alien. Ugh.
