You are bringing back memories of my insurance fights after Calvin was born. "Are you seriously trying to argue with me about whether or not I had a baby?"
Insurance policies are like airline pricing. It's like, "It's cheaper to fly from NY to LA, than from Austin to Little Rock? WTF? That makes no sense!"
Yeah, right before my medical drama, we briefly considered switching from our PPO to the HMO, but the whole referral thing made us change our minds. Sorry you are having to deal with this right now!
The insurance companies don't always make things easier for people that need their help. My two-cents on this issue is that we actually need to relieve them of some of their burden of regulation. A lot of their onerous policies that make things difficult for us, their customers, are put in place to protect them from the huge volume of regulation imposed by the government. Poltitians love scoring points for themselves by saying "I did good things for the voters by making sure the insurance companies are under control". But if we let the insurance people (who know their business better than anyone else) run their own affairs without interference, and use the fraud-prevention and general regualtions for business that already exist to protect all parties involved, they would be free to streamline their processes and respond more accurately to what kind of service their customers desire. But that's a hard sell when people have experiences like yours.
Oh, and I hope you're feeling better!! Stressing about insurance crap doesn't really help the healing process. I'm glad you've got our community to help take care of you!
