I seem to have overlooked Croatia's possible entry into mixed doubles with Martic partnering Marin Cilic with a combined ranking of 63 (this week). The Polish pair above would have a combined ranked of 57, but I doubt a wild card would be given to a country already represented in the field. Bulgaria, Belgium, Chinese Tapei, France, Switzerland, and Ukraine could all potentially enter teams; however, with the exception of Switzerland, the combined ranking of such a team would be over 95.
