Okay, now that I've watched The Reichenbach Fall three or four times, I realize that there are some things that are bothering me...

cut for spoilers...REALLY BIG SPOILERS. Especially if you haven't seen the ep yet... )

1. Molly and Sherlock have the conversation about "you look at John like my dad did when he was dying when he thought no one was looking" in the lab at Bart's when they're analyzing the footprints. But this is way before Moriarty has revealed his endgame. So then why does Sherlock look like this?

I thought that being a supergenius, Sherlock figured out as far back as the pool that Moriarty was going to take him down. And the endgame kicked off with the Crown Jewels heist. So when Molly and Sherlock are talking in the lab, Sherlock mightn't have had the Plan B for the fall exactly figured out, but he knew he was going to need a Plan B and that that would involve Sherlock's death, or at best either leaving John or dying or the end of his relationship with John. Molly saw that look on his face and knew she was looking at a man whose end was nigh, IMO.

2. Why is John upset about the media coverage?

In episode three, he's embarrassed at being called a confirmed bachelor because it sounds like he will never have another romantic relationship, and also there's an element of too much, too fast. Witness Kitty in disguise as a fan in the bathroom at the court house - more media, more harrassment.

3. In Baker Street, before the police come to arrest Sherlock, he accuses John of believing Donovan and thinking he's a fraud. Does he really think that?

I think he's upset and wants reassurance/to blow off steam.

4. What's with the case Sherlock is working on just before Moriarty breaks into the Tower? He says, "Henry Fishguard didn't commit suicide; the Bow Street Runners got it wrong."

I think the reference to the Bow Street Runners is, as you point out, and as John points up with his 'pressing case?' comment, a case so cold that it's permafrost. If it were in the book, which it's not, AFAIK. Fishguard, to me, is a ferry port. So if the dummy is a clue, maybe it means something like: dummy, I'm overseas? :D

5. Who arranges the call to John with the "news" about Mrs. Hudson?

I thought it was Sherlock. And I thought he thought that John would come back when he did; he probably had the sightlines worked out and everything. I imagine Sherlock chose the roof instead of the basement (for example) to make Moriarty choose a jump off the roof instead of, say, an overdose or a gunshot as the method of Sherlock's 'fall'.

6. How much of Moriarty's game does Sherlock have figured out on the roof? He sounds surprised about Moriarty's plan to have him jump, but of course he's not, since he's already put his own measures into place. Does he know the key isn't real? Does he know about the hit men? Does he think Moriarty is insane enough to kill himself? What's real surprise and what's him playing Moriarty?

I think he thought the key was real. I think he did suspect about the hit men (considering Moriarty had snipers at the pool) but not that Moriarty would top himself and therefore make him have to carry out Plan B.

7. Does no one on the police force think of looking for Sherlock at Bart's? He is a fugitive. Wouldn't that be the first place they'd look (after Baker Street)?

I find it a little hard to believe that he'd just think, "oh, he's going to get me" and not try to do anything about it. Although maybe he is and he just realizes that that's going to mean that he has to make John think he's dead.

I don't think he just accepted it. One can see the end coming and still hope, still plan, still try to win. He was definitely trying to outwit Moriarty right to the end, IMHO. But in the final analysis, Moriarty had more leverage, fewer principles, and a large number of snipers. :)

Due to the fact that Sherlock is not a murderer, the playing field between him and Moriarty is not a level one.

4. Fishguard is a ferry port in London? Maybe that is a clue!

It's a ferry port on the coast of Wales. One of the main routes to Ireland.
