I'm not exactly a scholar, but it sounds like pitiful has something to do with pity.

[She hears it. She hearts it and it makes her seethe. Buffy, having just recently experienced a momentary blaze of her familiar strength when she had earlier broken Doctor McCoy's arm, can even more acutely feel her current inability. Any call to action is therefore paled over by thought.]
