She spent more money on dresses and toys than on food, and dreamt of the life she would give to her little girl. But Miwako was not born female; the term the doctors used was "45/46 XO/XY mosaic". She was intersexed, more male than female, but because the doctors who looked at her determined that it would be easier to alter her into a girl than a boy, that's what was done. Miwako was raised as a girl and given regular supplements of feminine hormones, though she thought it was just ordinary medicine.
Miwako grew up as a more or less ordinary girl. She loved to wear dresses and wear makeup, she liked flowers and pink things. She did enjoy the occasional boyish activity, but she ignored those interests because of how much her mother praised her for the simple girly things she did. She never had any idea that she wasn't "normal"; it wasn't until she was sixteen that she realized that even though all of her female classmates were having regular periods, she'd never even had one. She consulted with her mother, who was oddly defensive and secretive. She insisted that it was perfectly normal to start so late, but when Miwako left the room, she overheard her mother making a phone call in a hushed voice; the words "--don't know how to tell her--" were heard.
A couple of days later, while searching through the drawers in the kitchen for something unrelated, Miwako found something shocking in a high-up cupboard that she'd never used before. It was a medical report that explained, in detail, her condition. She confronted her mother and father, but they denied any knowledge of it, and when she went back to look again, the file was gone. Miwako did some research online, looking up the terms she'd read in that file, and found out exactly what it was that they meant, and she found out something else as well: The "medicine" she'd been taking her entire life was a feminizing hormone, one with a side effect of causing extremely brittle bones. She'd always been frail, always been cautioned not to do any strenuous activities or she'd hurt herself, and this was the reason why.
Miwako sought refuge with an intersex support group called Shizuku no Kai. Her parents refused to discuss the issue with her, so Shizuku no Kai was the only place she could go to find help. In the meantime, Miwako's mother continued to force medicine and doctor's visits on her; now that she knew what they were all about, Miwako tried to resist going, but she always gave in in the end. She stopped going to school out of shame. And then she met Haru Hoshino.
Haru was also intersexed, and had been raised as such. Mr. and Mrs. Hoshino hadn't believed in forcing an identity onto Haru, preferring to let their child be the one to decide. Miwako felt a number of conflicting emotions toward Haru: Jealousy, envy, idealism, and maybe even love. Haru, meanwhile, was naive and didn't understand Miwako's predicament until she spelled it out, loudly and angrily. She couldn't believe how clueless Haru was. But on the other hand, she wished she could be as blessed, that she could have parents who respected her for who she was.
Miwako and Haru grew close despite her outburst, and despite her outing Haru's intersex condition to other students at Haru's school. Haru tried to talk to Miwako's mother, who would have none of it. After meeting Haru, who was proudly open about being intersexed, Mrs. Ihara got more nervous and decided that it was time for Miwako's "final surgery", which would finally make her "a real girl". Miwako protested. Having witnessed Haru's openness, she had gotten stronger. She didn't want to undergo surgery or become the perfect little housewife her mother wanted her to be. She ran to her room to be alone, only to hear the sound of a lock clicking from outside. Her mother had locked her in, and said she wasn't going to let her out, except for school (if she wanted to even go), until the scheduled day of surgery.
Miwako rebelled. She grabbed a pair of scissors and lopped off most her long, flowing hair. Her mother heard the noise and unlocked and opened the door, but by then it was too late. Miwako embraced her intersexuality; she was female and she liked being female, but she was also I.S., and it was nothing to be ashamed of. Unlike Haru, she wasn't going to come out to her entire school, but she took pride in who she was. She finally returned to school, having to repeat many of her classes thanks to having skipped for so long, and she was shy and kept to herself while out in public, but it was a start. She could finally return to the world as herself, rather than who her mother wanted her to be.

Sorry for being such a pest; I'm used to games with less-stringent requirements, so I didn't really put my all into my first drafts. Here we go!

It's clear that she never intends to make anyone feel bad or hurt their feelings, but she can't help blowing up. Once she realizes what she's done, she'll get quiet, even run away to hide if possible.
She hasn't had many people accept her for who she is, so she gets a bit clingy and desperate when she finds someone who does. She can be frighteningly passive-aggressive; when she and Haru discuss Haru's crush on a boy named Kenji, she encourages Haru to go for it... because she's sure that it would end in disaster and Haru would rebound onto her. She's not a bad person at heart, but she can't stand the thought of being alone and she still has an instinctive fear of being rejected because of her body, so she does what she believes she has to.

It was raining. Miwako didn't like the rain -- it made her hair cling to her head and weighed her clothes down -- but she had to walk through it to get home, now that she'd started attending school again. Her mind was racing, as usual; today had been better than yesterday, which had been better than the day before. It did get better the more she pushed herself. Maybe, eventually, she would stop trembling when she spoke to people. Maybe someday...
Her train of thought cut itself off. She was looking down at the sidewalk as she went, so she noticed the box and was able to keep from stumbling over it. Curious, she knelt down and picked it up. There was a little plastic window on the top, through which she could see...
"A cake?" She looked the box over -- it was from Haru's father's bakery. Of course it was; that was the closest bakery from here. But that didn't really explain why it was sitting out in the middle of the sidewalk.
Miwako suppressed a smile as she realized what this meant. She'd have a perfectly good excuse to go there and ask about it -- and Haru should be working today, too. What a pleasant surprise.
