Yes. Kobayashi Yosuke was dead.

You are correct. Sakurai Kotono and Amamiya Minako were both female personalities.

No. He was a personality inside of their head. And then he died.

Mm... not really. They didn't come out very often. When I was alive, Kobayashi-san was the dominant personality and everyone else was hidden from him.

Ceased to exist.

I don't know how. Kazuhiko said that Shinji was to blame. Shinji never talked about it.
