FDA alert on oral contraceptives
Some birth-control pills from Quality Pharmaceuticals were rotated in their package, resulting in reversing the order in which the pills are to be taken (which doesn't work so well as contraception). Affected products include the brands Cyclafem, Emoquette, Gildess, Orsythia, Previfem, and Tri-Previfem. More info here.
