Generally, the baby is squeezed until the head pops free, often this requires making a big cut in the vagina to increase space. Once the head pops free the doctor, mid wife, med student, cab driver, sucks some the of gunk out of babies breathing parts and then pulls it the rest of the way free by pulling it down and then up to dislodge the shoulders. For a first born the process of moving the baby out can be very slow. I actually wonder if with the advent of c-sections we will select for larger babies until none of them will escape properly. Babies are also larger with diabetes and that is rising as well. People who have had babies before can squirt out a baby with relative ease and low injury to themselves. This was not the case for one patient who had a baby over six pounds followed 11 years later by a baby over 8 pounds. The baby wouldn't come out so she went to c-section. Having a baby is one of the most dangerous things a woman can do, it can be a very life threatening situation. Sometimes the placenta poisons her and she could die if they don't deliver the baby, for example. We've made it much safer because there is so much medical involvement.
