basically you have two options.
1) Fill them in yourself/with your partner
2) Get someone who knows the forms to fill them in on your behalf.
If you think option 2 looks like a good idea, that's because it is. :)
Again you have 2 options.
1) Ring the DSS office in your area, tell them you are disabled etc., can someone come out to you and help you fill the forms in please?
2) Ring a local disability advice centre, they will tell you who can help and the local CAB usually has someone to help with forms.
Our first application for DLA we did ourselves, at appeal it was thrown out on a technicality.
After applying a second time and getting the local disability adviser to complete the forms (she also went to the appeal with me) they decided that the first EMP report was innacurate and we needed more med evidence. This has been collected and we go back for the final hearing (fingers crossed) in the middle of November.
Whatever the forms are, DLA/Incapacity, get you GP to fill in the section from someone who knows you as they cannot ignore a Docs evidence, other supporting evidence from ex work colleagues and friends also helps.
ALWAYS CHECK THROUGH THE FORMS YOURSELF BEFORE YOU SIGN THEM. IF YOU DON'T AND YOU MAKE A MISTAKE THEY THROW IT OUT JUST BECAUSE YOU CAN'T COMPLETE THE FORM!
