This is superbly written. The imagery is wonderfully apt - I especially liked the idea of the "trenches of emergency medicine" in which John has taken cover from his grief. The dream sequence had me in tears. But I must say this feels more like the first chapter of something, rather than a completed piece. Please say there's more to come! I for one would adore for it to continue.
