I'm new around here and I don't know if this has been done or not (still skimming over the other parts) but I propose a fic like this:
A couple and their son have an car accident. They all are hushed to the hospital, the father dies just as they arrive the hospital and Doctor Watson tries to save the mother without success. Only the child (boy - girl can be fine too) is left alive.

The boy apparently has no other family and until matters with the Child Protective Services are resolved he has no where to stay (idk how this stuff works so bare with me). Feeling guilty because the surgery on the boy's mother didn't go well, Doctor Watson insists they boy should stay with him for the time being.

John brings the boy home and Sherlock's not pleased at all. They only have two bedrooms and because of that John has to sleep with Sherlock so the boy can have the other bed.

Bonus point if the boy is snarky - because he's grieving his parents deaths - specially towards Sherlock (since he's insensitive towards this). Also more bonus points if the boy points out how gay they are for sleeping on the same bed and other gay remarks.

tl;dr just make John guilt trip into something and make him have to take care of a child for a certain time. Sherlock doesn't like but eventually they both get pulled in a parents' role or something. The child is snarky specially against Sherlock but feels some affection towards John since he's taking care well of him/her, wtvr.
