Something else occurred to me: there's usually a lot of fandom speculation going around as to what overarching seasonal plot threads the plot of the episode is a parallel for. Often this is very clear, as with the Cain episode (Dean=Cain, Sam=Abel, Cas=Colette) and what the point is, but this week has me stumped. Going by the classical logic of siblings plus one, Cas would be spa dude, Dean would be spa lady and Sam would be spa lady's brother. But what is this mirror supposed to show us? Sam is hardly a vicious monster past saving, and it seems unlikely that he would have reason to kill Cas these days, either. Maybe the parallel is simply that Dean is going to be alone in the end, with both Sam and Cas dead? I have honestly no idea. Thoughts?
