Someone has finally decyphered the Quantum Link network for Commodore 64s. It is called Quantum Link Reloaded, and you can view information about Q-Link and QLR at www.quantum-link.org.
This means that you can, with their "easy install," download a C64 emulator that will automatically go to Quantum Link. Just hit F1 when it says "Make your modem connect" or whatever. (The green screen.)
The profile your requested to fill out is not for billing purposes, and can be entered with whatever.
