advice?

Hey all, I have a performance review coming up this next week before I go full-time. I've been doing a good job (as verified by my boss's boss) and I'm underpaid for my field, so I am pretty sure that I will get a raise.

However, what is the best way to approach this? I have never asked specifically for a raise before and that is kind of the point this time, so... I want to do it in a way that A) sounds like I am used to this sort of thing and B) will maximize my potential income increase (AKA MORE MONEY :)). And as well, what else should I be prepared for in a performance review? Should I come armed with a list of all the things I've been doing? Do I do any talking, or is it mostly listening?

Any input is appreciated. :)
