Yeah, I think a better translation for "tono" would be "lord" (or "m'lord" as you said, since it seems to be an old word). And I only know of this because I watch Samurai Sentai Shinkenger.

How you translate dono depends on the context. It's an archaic word -- a word that isn't used anymore. Basically, it means feudal lord. So, in a period story it's used out of respect or humility. Kenshin in Rurouni Kenshin always used "-dono" when talking to people.

So, when the twins use "dono" in Ouran Host Club, they're using an old-fashioned title to mock Tamaki. Since he thinks himself as the "King", they call him "Your Highness".

It's the same as the episode of Lucky Star where Konata started calling Kagami "Kagami-sama". Except that Kagami was hugely embarassed, while Tamaki is totally oblivious to the fact that the twins are mocking him.
