Where: Outskirts of town
What: Rukia's back, but isn't waking up... Well at least not yet.

Unlike most who had had their hearts returned, the girl wasn't waking up. She returned and was breathing, but wasn't awake. Outwardly, sure, she could have been sleep, but that wasn't the case.
Rukia was still unconscious.
Outside of the rise and fall of her small chest to note that she was breathing, she wasn't moving otherwise. Her reiatsu returned with her, but only being very, very faint. Those who had a sense for presence and spiritual energy would note her return. The message on her LP would note otherwise to those without the ability.
Even for Rukia and those who knew her, this was an unusual situation. If someone was malice enough to want to take her life, the shinigami wouldn't go down without a fight. But her clothing was not tattered, her body showing no signs of a struggle or bruising.
This situation was too weird, and too off. Not to mention that she was in her shinigami outfit and she could be visibly seen. The only people who could see her in this form were other shinigami, ninjas, and well now, whoever attacked her. Something happened with her heart being taken.
