10 Signs you have it.

1. You wish so-and-so would stop talking about what they did that day so you can tell them about what happened on Sims.
2. When someone tells you they achieved a goal you automatically picture the Lifetime Happiness points being awarded. This is especially troublesome when people actually achieve goals that are included in the Sims, such as do a cardio work out for an hour, because then you picture EXACTLY what happens in the Sims.
3. You can no longer do things like eating, using the restroom or riding in a new car without picturing the mood-boost icon in your moodlets, as well as the green status bar restoring itself in the needs section.
4. You're more excited for a Sim accomplishing a life goal, or aging up, then you are about things you accomplish during the day.
5. The Sim you created for yourself starts to feel more real, and you refer to their children as your own. As in "Oh did I tell you first-born learned how to talk today?" or "Second-born is going to age up in another day or so, I can't wait to see what they look like"
6. The more you play the more you lose track of what time of day it is or what day of the week it is, because you are operating on Sims time.
7. You forget where you put things because your mental layout of where you live has somehow merged with the layout of your main Sims family's house.
8. In a moment of absent-mindedness you catch yourself looking for a favorite shirt, or outfit, that you never owned and you realize you were picturing what your Sim is currently wearing.
9. It drives you nuts when someone leaves your house/apartment because you can no longer follow their needs and moodlets and make sure they are doing everything they need to be doing.
10. Whenever you do something like writing, cooking, working out or reading you find yourself wondering how long it will be until you level up in that skill, and you picture the status bar above your head.
