...I don't want to divulge the exact details publicly, but...I would have to say it's letting someone very important to me be harmed out of spite towards him.

You can't beat yourself up over these things. Sometimes...things happen, and we can't quite forsee them, but that's not because we are stupid. It's because we...or rather, you....are human.
