I think it was YourLibrarian who hypothesized that Sam recognized Meg the same way that Dean recognized Alistair: based on just how twistedly intimate their relationships were.
Methinks it would be worth a meta to compare Nick and Dean, btw. Their situations, their choices, the ways their consent is sought. *thinky*
