[Once Riku gets to the clinic? It's pretty easy to figure out where Hope is - the crystal he holds glows brighter the closer it gets to the boy in question. To be more specific, it's as if it gets warmer, sensing the holder. As for the other, he'll be in one of the empty clinic rooms, almost still, unresponsive to many.]

[And it really doesn't take him that long to find his way there. Though he'd never been there himself, maybe it was a combination of Kairi's guidance and his sixth sense that led him right to the location. Well, at least until the crystal begins to glow and get warmer. ...Huh. So this was what a small part of its owner's world was like.
Riku approaches the clinic room, knocking on the door lightly before entering. He can see the other boy lying there... and the flashbacks sting.]

[When the door opens, there's hardly a response. To his name though, he manages to sit up and the crystal begins to hum in response to his presence. Hope's expression is more than confused, and it's as if he doesn't even register his crystal is in the room.]

[There's a faint smirk. So... at least he can sit up. He walks over, holding out the humming crystal to the almost comatose Hope.] We found this while taking care of the remaining Heartless last night. I was told it belongs to you?

That's- [Reaching up, the crystal glows a vibrant yellow-green and the haze over Hope's eyes fades.] -y-yeah, it is. ... It's okay.
[The crystal hovers in Riku's palms, then hovers over to Hope.] T-thank you.

[He lowers his hand when the crystal's returned to it's owner, nodding his head.] No problem. Sorry it took so long to get it back.

The Heartless are the darkness made real; what's left of someone's heart and soul when they either give in to their negative emotions in my World or fall victim to the Heartless themselves.
