I started with Kotiya, but have since been using Kochiya as it's from the more commonly used form of romanization. And honestly, hepburn romanization just SOUNDS better. Anyhoo

Her personality doesn't change. It develops. The changes aren't as extreme as the fandom jokes them to be. She's just adjusting to life in Gensokyo. And if anything, Touhou 12.3 is a nice correlation between how she is portrayed in the previous games, so I'd use that as your base (assuming you are playing her from that canon point). Generally, she really is a nice girl, but she gets VERY enthusiastic about doing her job, and can sometimes (read: pretty much all the time) get highly carried away. I'd suggest doing a script read-through for all four games she's appeared in so far, as well as the comments about her from both tengu in Double Spoiler (There's also Grimoire of Marisa, but it doesn't really comment on her personality). That should help you build a bigger picture of how she is.

Sanae's past is hinted at in Alphes's promo art for Touhou 12. In this art she is depicted wearing a common Japanese school uniform, suggesting that when she came to Gensokyo, she was still in her teens, if not middle school age. PERSONALLY, I go for teenager. It's also suggested she grew up near Lake Suwa, the location upon which Suwako and Kanako are based. Now, Lake Suwa, in spite of being home of the largest Shinto shrine in the world, has a somewhat sparse population. So she probably didn't have a lot of exposure to city life. Whether or not she had any friends back home is debateable, but it would seem she had a keen interest in anime/manga. So maybe she was an otaku? Who knows. But that seems like a good interpretation.
