http://yugioh.wikia.com/wiki/Akiza_Izinski Changes from canon, if AU: In the Yugioh 5Ds world, there was a man who had tried to build a bridge, which became known as the Daedalus, between the ghetto Satellite and the upscale Neo-Domino City. That man was hunted down for daring to change the social strata enforced after the events of Zero Reverse, and when he was cornered, he merely rode his D-Wheel to the end of the incomplete bridge and flew off, disappearing into legend. In canon, that man was Rex Godwin, the brother of one of the Signers, who ultimately manipulated events to try and bring the Signers together for his own ends - and also, by expanding the use of Momentum during his tenure as Director, helped to inadvertently set up the future that Z-ONE and his disciples were from, where all humanity was destroyed. But here, that didn't happen. Rex Godwin truly did die, drowning in the waters that separated the two halves of a once whole city.
That single death changed everything, even more than the millions of deaths during Zero Reverse. Without Rex, the arm of the Fifth Signer floated away, never to be seen again. The other living Signers were never manipulated into meeting each other - thus, the Signers never would meet despite the marks they would all inevitably wear as the Crimson Dragon tried to revive itself and its bonds. The Dark Signers decayed and died quickly without their vengeance against the Signers to sustain them - as no Signers were revealed for them to fight. And the future of Z-ONE never materialized because the energy of Momentum was ultimately considered too dangerous to use and research on it discontinued.
This, of course, didn't mean there wasn't a threat to the world. In fact, of all of the organizations that the Signers fought against, it was Arcadia Movement who has benefited most of all from this slight change in the course of reality.
After Aki Izayoi was accepted into psychic duelist haven of the Arcadia Movement, no Yuusei came to save her. She never saw another mark like hers. No one was around to try and temper the anger that Aki's leader, Divine, deliberately encouraged. So her anger became greater, and so too did her power, which she poured into the literal and figurative mask she wore as the Black Rose Witch in order to deal with her feelings and actions. There was absolutely no doubt that Aki was the strongest psychic duelist in the world, and that her ever-increasing power was causing more and more destruction. First, among the populace of Daimon Area. Then, slowly, Divine encouraged her to attack places here and there in Neo-Domino proper. So she did, in the naive belief that in the end it was all for the good of Arcadia Movement - and that, perhaps, Divine would finally give her what she craved. The love of someone, anyone, that would finally make her feel whole and not like the monster, the witch, she genuinely believed herself to be. Maybe one day the guilt and sadness she felt at her actions would be gone for good. Of course that day Aki longed for would have never come, since Divine was only using her to take over the world, allowing him to shape and sculpt her life and thoughts out, to the point where she trusted only him and did all he told her - just like a good puppet should.
Aki didn't know this, and would never find out just how little Divine cared for her and how much he was really damaging her. One day, in her destructive, fearful persona, the Black Rose Witch killed a bystander for real. She hadn't meant it. But the response was swift. The police for of Neo-Domino City used this death as an excuse to raid the Arcadia Movement headquarters. Dozens were arrested. Dozens more died. One of them was Divine. And that was that day Aki...snapped, and the Black Rose Witch and the tortured teenager became one. No longer was there a need to differentiate between the personalities in her crazed mind. They were both the same.
By the end of that rampage, which only ended when she dropped down exhausted and had to be forcibly dragged from the ruins by the remaining Arcadia Movement acolytes, thousands of Neo-Domino citizens were dead and entire portions of the so-called utopia were razed to the ground. But she no longer cared; her mind was warped with rage and desire - a desire for revenge. Humanity had taken away the only thing she had ever cared for. The only person who had given her something to believe in, a place to call home, and people like her. They were the true monsters, not her. And for that, there was only one thing that humans deserved, the one thing they had given to Aki: death and despair. And that desperate hope of someone loving her and accepting her, the one thing she'd always clung to - she decided it was the key to causing that despair. All she needed now to make everyone pay was her power, and anything else she could get her hands on and use at her disposal. Only the psychic duelists would prevail. And she would pick up Divine's bloody banner and lead that victory.
For the next nine years Aki set about to make this goal true. First she found a new place for the Divine Movement - what she decreed the Arcadia Movement's new name to be after Divine's death - and laid low for awhile to figure out what to do. Then, she enrolled in college, deciding to become a doctor as part of her insane, irrational plan. For the first five years of that time, she managed to keep herself from lashing out when she was in public in her normal persona, if only because she knew that biding her time would make vengeance all the more sweeter. She managed to get her degree in neurosurgery - perfect for what she had planned. And once she graduated, without warning, attacks by psychic duelists started up again. Only this time they weren't just in Neo-Domino City. They seemed to be everywhere, and in numbers no one could have predicted. There were even normal, non-psychic attackers who claimed nothing but undying loyalty to the cause of the Divine Movement - and it's leader, who they claimed would rule a new, 'pure' world.
No longer was this leader the Black Rose Witch, creature on the fringe. She was the Black Rose Queen, a figure front and center - and everyone will finally notice her at long last, whether they want to or not.
Personality: On the outside, and when she isn't parading as the Black Rose Which/Queen/whichever people want to use, Dr. Aki Izayoi seems - seems - to be a bit of an ice queen. She's distant, cool towards most people, and doesn't care much about what other people feel, even while performing surgery (which is the only part of being a doctor that she seems to enjoy or be fascinated by). She is also easily irritated by certain types of questions or remarks (and might snap a little if someone digs a little too deep), and doesn't mind being completely critical and rude when people mess something up or do not meet her expectations. Still, on the surface, she doesn't seem entirely bad - at worst, she's a bit of a bitch and she doesn't seem to mind being that to people. It just shows her to be truthful and honest, if a little abrasive. Right?
Unfortunately, nothing could be further from the truth. Aki Izayoi, in reality, hates humanity and is quite insane, in a crazy like a rabid fox kind of way - she's thinking enough to know she needs to not get caught and makes sure to try and not get caught. Because Aki hates humans with every fiber of her being. In fact she hates humanity so much, she would not have a problem killing characters right where they stand, right at that moment - and she'd probably smile and maybe even laugh a little while she ripped them apart. She believes humanity is unworthy of being the caretakers of the world due to their cruelty to those who are 'different', particularly towards psychic duelists, the 'monsters', who humans have killed and subjected to humiliation and isolation and despair; because of this, humans must be subjugated, and psychic duelists, who had only wanted to be accepted and loved by humanity before, must now take over and fix things. And in order to do this humanity and anyone who stands in the way of psychic duelists must be placed into total despair, either completely under her control or six feet under. She is the Black Rose Queen, after all, and nothing gets such a monster's jollies up like causing chaos and mass destruction and torture to humans and making them do things against their will. In fact, she's been doing things behind people's backs without them knowing about it, right under their noses. But Aki won't do anything too obvious that would actually reveal that she is the Black Rose Witch/Queen, even though she would absolutely love to and discard being Aki Izayoi forever. At least, not yet.
And the reason that everything she does now is to work towards this goal of destruction and domination? Love, despair, and how she ties the two together in her insane mind. It's an insanity rooted in events that occurred during her childhood, after her powers woke up and she had been accidentally called a monster by her father; since then that was what she identified herself as. More accurately, she was a "fragile monster" because despite her powers, she was emotionally distraught as she grew up into a teenager, when she still had her sanity, and some semblance of hope. All she ever wanted was love - love from a father, love of a friend, anything - to validate that she was something like a human, like a normal person. However, the more people rejected or seemed to reject her, the more her feelings of loneliness and guilt over her nature grew - and the more her anger at the world grew, as the world saw her more and more as a freak of nature as she got older and she found herself unable to control her powers due to her anger at this. By the time she was enrolled in the Arcadia Movement, her guilt over her powers was such that she had created the persona of the Black Rose Witch in order to handle her powers, which she 'set free' by deciding to laugh in her despair and enjoy the chaos she caused.
However, despite that declaration of just needing to 'laugh' (i.e. go slightly insane from the guilt) she still desired love and happiness from others, and she merely pinned these hopes on Divine. As a result, Aki had no self-esteem growing up, didn't learn to love herself, and despite what she might say to the contrary nowadays, she is still someone who lacks self-esteem, unable to love herself, because of her flawed belief that her self-worth is dependent on others loving her, even though her mind links love to despair. Divine, on his part, didn't really help her; by telling her to simply use her emotions to feel and live and to let him do the thinking for her, he managed to make Aki dependent on him to the point where he had a crushing psychological hold on her. He also had her believing she is the hope and future of all psychic duelists, and that hurting and even killing people in the course of using her powers were all acts of necessity for the good of the Movement, despite Aki's initial guilt and misgivings. It's this hold Divine has on her which dictates what she is now and why she is doing what she is doing.
Unlike canon Aki, who managed to see through Divine's hold on her and who had the Signers to help her fulfill her potential, Aki's original self and that small bit inside of her which cried for the help she knew she needed has been forsaken and buried very, very deep within her to the point where it would be incredibly difficult to unlock it again. Without Yuusei to pull her out of her despair, she never learned how to truly stand on her own two feet and make her own decisions, or to become truly confident in herself and break away from the negativity of Divine and what he truly stands for, or even to understand what real love was. Her entire existence hinged, and now still hinges on, how Divine would think of her, what Divine would tell her to do, and also how she must make up to Divine her perceived 'weaknesses' that she had showed when he was still alive. This includes 'thinking', though her current method of 'thinking' is really thoughts of her revenge and how to best pull it off.
Divine's death was, after all, the blow that twisted everything in Aki, including her ideas of love and despair being connected to the fullest. Now, she is fully convinced in the notion that her desire for someone's love led her to despair - first from her parents, then from other children, and finally from Divine, who, unlike all the others, must have wanted to love her (in her twisted mind, of course - why else would he have chosen her as the Movement's "light"?) and became a martyr, a saint, because of it. Because of that, nothing he did was wrong after all, not even his experimentation on his fellow psychic duelists - which she now has no problem doing and condoning, not only because she believes herself to be a monster in any case, no matter what, but also because of her insane belief that doing this will make them all stronger and make Divine proud (and perhaps, make him really love her despite the risks) if he were there.
Thus, in her crazed logic, this need for love, mainly for her to induce others to love her (as she has gone to the point where she knows, or believes, she herself cannot possibly feel any kind of love for anyone but Divine, but, in true insane logic even if she won't admit it, she still wants others' love even though she hates everyone and likes to hurt them) is the key to stopping humanity once and for all by bringing them to ultimate despair, rendering them unable to fight back. And if this involves waiting, she'll do that, because it doesn't matter how she pulls off her endgame (like her canon counterpart there is a reason she became a doctor, though her reasons have nothing to do with compassion...) or what benefits she derives from taking over the world in the name of the Arcadia/Divine Movement. In the end, it's all in Divine's memory and continuing his work - and the memory of Aki's love for Divine.
TD;LR This particular Aki has...issues. To say the very least.
Abilities: Psychic duelist/Dueling: A psychic duelist is basically a person capable of bringing to life the various cards of the game called Duel Monsters, including spells, effects, traps, the monsters themselves, and so on. They can thus use whatever card they bring to life to inflict real damage to people or to effect the environment around them. While it's not totally fleshed out, it's likely this power is called a "psychic" power because much of the strength that is brought forth from the cards - and sometimes even the type of power that results - seems to be dependent on the person's state of mind and emotional condition.
Aki is the strongest psychic duelist in the entire world, to the point where she doesn't even need a Duel Disk to materialize the power of the cards - all she has to do is think of what card she wants to bring out - usually they are cards from her Rose Deck (also known as the Black Rose Deck) and are aimed towards ultimately summoning Black Rose Dragon - and it will become real at her command. However, her power is completely destructive due to her unmitigated rage and hatred against humanity, and is hugely amplified by the length of her hair, which she has completely grown out since Divine's death. She has almost no control over this power whenever she decides to use it to its fullest, and as the Black Rose Witch has leveled wide swaths of city blocks just by summoning one monster or spell. The only thing which has any kind of inhibiting effect on it is a special hair curler, which she generally wears when going around as Aki Izayoi.
Neurosurgeon: Yes. This Aki is a certified neurosurgeon. Meaning she knows how to operate and do things to people's brains, especially implant things into them. Which is not a good thing.
Signer: Her Signer mark, which she considers a "curse", does have several powers and deeper abilities despite her hatred of it. It can create a shield which can protect her if she is in danger. It also reacts and sears into her arm if a Dark Signer is nearby. It also, interestingly, reacts when she is in extreme emotional pain, or when someone (like, say, Yuusei) tries to reach out to her or when someone with a bond to her is in danger. In a sense, the Signer mark is almost where the all of the broken, sealed parts of what was once Aki have ended up.
Writing sample: It had been a long day at work. Three surgeries, all of them long and difficult. She'd almost lost a patient. But she saved him, and was congratulated for it by the unit head. She'd only just completed her residency, but her skill was such, the head claimed, that he'd have no problem getting her a permanent position and a raise before long. She was, after all, the new star of the unit and the talk of the whole hospital.
Aki had ignored the praise. They were hollow words, coming from a human. What she'd done was merely practice for her real job.
The doctor walked, her top still blood-stained from the procedures, through the town, getting on buses, transferring to and fro until she was certain she would not be followed or recognized in any way. She finally got off a few blocks away from Daimon Area, making her way towards an unassuming apartment building. She didn't go in the front way; instead, she went through a secret passage inside what looked like a rusted trash compactor.
It was there on the stairs of that secret passage, and only there, that Aki brought the old, dark blue cloak around her body, and brought the mask - her true face - out. As soon as the mask came into place, the roller in her hair fell out of her hair, almost by magic, allowing her long hair to hang down across her.
"Black Rose-sama." The minute she was at the bottom of the stairs, a half-dozen men and women, all wearing the cloak of the Divine Movement, bowed obediently. Behind them were three people, unconscious and hooked up to IVs on operating tables. "Welcome."
She was home. And now, as she looked at the three on the table, she cannot help but smile underneath her mask. These three humans survived the tests. Perfect. Divine would be pleased.
It was time to operate.

When I come into work in the morning, they say hi to me, no matter what the circumstances. They see me, Izayoi Aki, as someone of stature, someone who is trustworthy and dependable. When the doctors and nurses come to my side, they look at me with awe, wondering how I have come so far in the world. When patients see me, they smile, grateful for my talents. They think I am nothing but a normal person, a good person.
How I loathe them all.
So many times, day in and day out, I have stopped myself from destroying them all, from razing the building to the ground and setting the sky ablaze. How I desire nothing more than to see those smiles turn into horrified glares, and those greetings of respect become screams of terror. They are human, after all. If they knew anything about me, about what I truly was, they would come to hate me. And they would try to kill me. So it is only fair I hate them in turn. After all they have done, that is all I have left for them.
They'd say I was crazy to want them dead, to want to hurt them. But they wanted Divine, and all of us, dead. So, I'm only returning the favor. I'm only doing what must be done for my people, the people of the Movement. These normal deaths would be forgotten by the other humans anyhow. The cruel humans who know nothing of true suffering...true despair...the curse of being different...of being...stronger...
They'll know. They'll all know. But it's not time yet. I'm not ready, this light, the Movement...not ready yet. So for now I'll wait. And the humans will never know what is coming. Because it's only fair.
They started this war. I..the Black Rose Queen...will finish it with a smile.
