In the beginning, something happened, and because of that, the rocks fell.
They fell everywhere.
Seriously, everywhere.
They even fell upwards into leviathans like Moya that live in space.
The rocks fell on Lucifer before they fell on Dean Winchester, so there's that, at least.
Those rocks killed everyone everywhere...except for the Master, because in Time Lord tongue, "Master" is pronounced "Nemo," which means "No One."
So, because the Master's name was No One, Toshiko naturally wasn't killed by the falling rocks. Oh yeah, I forgot to mention that Toshiko Sato is the latest regeneration of The Master (he's very edgy, that Master, refusing to always be the same evil man).
Yes, I the Narrator know that Tosh longed for Owen and probably boinked Ianto and Jack Harkness - didya think the Doctor was the only man in his (her!) life?
And then the Master Toshiko gave birth to the most awesome and beautiful baby that had ever existed.
And, because babies love and trust their mothers unconditionally and absolutely, she was able to use that power just as the Doctor had done on the airship in the Year that Never Was - and the Master created inhabitants for all the worlds that would worship this beautiful baby.
