if memory serves, demons will use any body opening (except skin pores, oddly) to enter a person & take then over.
you'd think Sam would know this.
I mean, if not for Lillith saying she wants Sam and Dean's heads on pikes, I'd suspect she was planning on implanting part of herself in Sam. then again, maybe she is - a contingency plan, in case anything happens to her herself.
