[Varric wasn't one to pry, bu when a co-worker suddenly came in with bandaged hands and happened to be chain smoking then it may possibly be a concern to him.]
Sure looks like you took a beating. Maybe problems a shrewd dwarf merchant can help you take care of?
Did I ever tell you about the time an associate of mine got his hands broken by a couple of humans who seemed to be angry he was selling in an area they deemed theirs?
They ended up mysteriously shot through the head with crossbow bolts. Tragic, really.
[Varric's simply making small talk at this point, he can easily tell Lancer is in a pretty bad mood. Varric being Varric, he just can't leave well enough alone, though.]

[It was admittedly a welcome distraction.]
Don't think anything that drastic is really necessary--as long as I can hold a lance I don't really care what state my hands are in.

I guess an expert in his trade doesn't have a problem doing anything with limited use of the hands, though.
How about I test my skills and fix you something? It'll sure numb everything out.

[Varric gives a shrug, and takes a long drink out of one he had made for himself.]
What was your name? I'm actually not sure if I ever asked or found out.
