Some rats do develop a permanent head tilt after an inner ear infection. I had a rescue girl who was quite tippy but it never bothered her co-ordination wise, I miss that little love ;-;. Anyways it's more likely that this is happening, rather than a PA. Definitely take him back to the vets and see about trying out a different AB. They usually try Synulox/amoxycillin after Baytril, or doxycycline, like you said(or a combo).
An inner ear infection really needs to be treated for at least a month. I think he needs a combo like baytril and amoxycillin or doxycylone. I would also add a steroid like prednisone since his eye is bulging, it will reduce the swelling and give the antibiotics a better chance and working. I have also heard Orbax giving decent results for ear infections and Im pretty sure it can be used with Doxycylone or amoxycillin too. You still have options. Also he me always have a bit of a tilt. It should lesson though.
Our McKay has a tilted head, we think from an ear infection. His son, Lorne also has a tilt, which is /far/ more pronounced. They both do just fine and McKay is nearing (if not already) three, his son will have his second birthday in February. Both are fine and perfectly happy. They were both on meds for a month at least (Lorne has a wheezing problem that has never gone away) and it didn't do anything to change the tilt or anything else, though it cleared up McKay's ear infection.
I don't have any advice but I hope things get sorted out. He's a cutie! If you haven't gotten it figured out the rat shack has some great folks that could probably help.
