You'd be great on How Do I Look because I think you'd laugh your way through it... it's possible to go through that show's process and just have fun. I think it gets personal only if the makeover guest allows it to. I mean, when the guest cries when they throw out an old ratty sweatshirt... come on, it's just a piece of clothing, not an album of old family photos.
I think people sometimes lose perspective. Why not just have fun with it? Why not look a little better if you can? (without breaking the bank or losing your mind, of course)
And if I were an accomplice, I would have fun with it too. I would use an extremely light touch... I would NOT go into the "you'll feel better about yourself / people will treat you better" psychological bullcrap. Because it's just clothes, and it's just fashion, and it should be fun, in my opinion.
