Not sure how this happened, but I went to the sign-up summary on AO3, and both "Eighth Doctor/Jack Harkness" and "Eighth Doctor/Jack Harness" appear on it.

If you intend to request the relationship, I suggest you use the correctly spelled one, but if you intend to offer, the best option would be offering both.
