It's probably good for her that she's startled him into dropping the knife. Better that it manages to not really hit anything on the way down, aside from the flat of the blade bouncing pointlessly off his shoulder as the thing falls.
He ignores it entirely in favor of wrenching his hand out of her grasp, though he doesn't move for it. She hasn't warranted anything quite yet, and Giovanni probably wouldn't allow that to get very far, anyway. And he knows better than to continue trying to hack at the damn ears while these two are here.
...of course, once he sees who's with her he smirks. More than a bit. He aims his words at Cynthia, though.]
