To locate and update In-Character (IC) medical records easier, I've decided to make this organizational post.

How it will work:

1. Your character fills out the medical form ICLY.
2. I comment with the information given under their FIRST NAME.
3. If information needs updating or noted, a reply can be submitted as a comment to the character's

3. Name of place of origin? Demon World but have taken residence in Human World for many decades.

7. Medical history? Sprained wing twice.
8. Have you gotten sick since your arrival here? Yes, a few times. Caught a cold, and also came down Atropos but only suffered flu-like symptoms.

10. Have you been in Discedo before? Yes
A. If so, how long was your stay? First time was 2 weeks, second was 2 months, this last time has been over a year.
B. How long were you absent before coming back? First time I was absent from Discedo for 7 months. Second time was for one week.
11. How many times have you died here? Twice.
12. Do you currently have your chip? No.
13. If not, then how long ago was it removed and by who? Last removed by Chopper back in November. (2010)
A. How did everything go? Did your powers effect the city? First time it was removed magic powers affected entire apartment building causing levitation and animation magic to run wild. I also was stuck as my "older" looking form for three days. Second time my transformation magic only seemed to not work correctly and I was stuck in my dog form for days.
14. What abilities do you possess? Transformation magic, Aviation magic, Portal magic, Fire magic, Animation Magic (making inanimate objects move), and Levitation magic.
