teethbrushing

Given the need to accomplish teeth-brushing of infants/toddlers, do you (personally, not generically--I'm looking for anecdotes, not advice per se):

*Let the child do it herself and perhaps do a not very good job, but without any emotional upheaval
*Try to help the child do it with her cooperation and a minimum of screaming
*Sit the child on your lap and brush the teeth well yourself despite the screaming howls of anguish
*A combination of the above
*Something else

And,

*When did you start using children's toothpaste? So far we're just using the toothbrush and water.
