The flashback is from in-between episodes 3x16 and 4x01. Sam while Dean was in Hell.

He doesn’t really know how they ended up in this position. It’s probably one of the last things he should be doing yet somehow he can’t seem to care, mostly because it makes him forget but also because where else would he rather be? than here, Firmly pressed between the legs of Brooke Davis.
Her grip on him is strong, one hand gripping fiercely onto his shoulder as if holding him as close as possible and the other pulling at his hair, directing his mouth exactly where she wants it. They have been in this position for the last fifteen minutes and she can’t seem to drag herself away long enough to tell him how much she missed him.
It’s not difficult to assume he missed her, considering he plopped her on Bobby’s kitchen counter and he silenced her with his lips. His hands are now placed firmly against her neck, one slowly moving to cup her cheek as her tongue enters his mouth in order to match his eagerness.
He reluctantly pulls back when he hears the front door open but doesn’t move from his position “That would be Bobby.” His voice is full of disappointment as his head exhaustedly rests betweens her neck and shoulder “We can finish this..Later..” Placing a soft kiss on her neck before pulling back and helping her off the counter.
She stares up at him, her hand still holding his before removing it to straighten out her top just in time for Bobby’s entrance into the kitchen “Dean I got us some....”
“Hey Bobby.” Brooke greets him with a sad smile before closing the gap between them and hugging the older man, he is shocked to say the least. Dean had told Bobby not to call anyone and that they would deal with this alone, as a family.
“Brooke Davis.” He smiles and pulls back from her “Pleasure to see you...How are you girl?”
“I’m....good...well as good as you can be during the apocalypse” She shrugs and he lets out a small chuckle, same old Brooke.
“Good to hear. Dean can I talk to you for a minute? Alone!”
Dean stares at Bobby in confusion before nodding his head “Be back in a minute, stay here.” He tells Brooke with a small kiss on her forehead and then continues to follow Bobby outside.
Brooke watches them go, her head popping around the corner in time to see the door close before tip toeing across the hole and down into the basement. She slowly makes her way to the iron door and reaches her head up to look through the glass. She spots him in the corner, sat helplessly on the floor, his back to the wall and head in his hands. Without hesitation she opens the door and steps inside.
“Sam?” She shuts the door behind her and he slowly looks up at her tiny form. The black in his eyes scares her deeply and she finds herself stood with her back firmly to the door and hand never leaving the door handle.
“Brooke!” He stands and makes his way over her to her, She puts up her hand in one quick motion and he stops dead in his tracks. “Hey..”
“Hey. Please...Please stay back, I just want to talk to you.” Her voice is full of fear and his forehead wrinkles sadly at the thought of him scaring her like this.
“I won’t hurt you Brooke, I don’t know what they told you but...” She crosses to him and cuts him off with her hands settling around his waist and her head buried into his chest. He soaks into her and pushes his head into her hair, bringing her even closer to him “Hey..” He repeats softly as his hand settles into her hair.
“I missed you.” She whispers as she pulls back to stare up at him “You just took off..”
“I know...I know I’m sorry.”
“Sam? Sammy!” She exclaims putting down her glass collection duties and jumping into his arms, she stays there for a second before pulling back and looking behind him.
“Brooke...” He begins slowly, his voice cracking.
“Where...Where is he?” She asks as tears begin to form in her hazel eyes, She watches as his gaze hits the ground and her hands push him backwards harshly “Well?”
“He’s gone Brooke. He....He’s gone. Lilith...she.. got him.” His eyes are filled too now and he pulls her back into his embrace before guiding her outside to the car.
She moves from him and leans against the Impala helplessly. Her arms wrapping around her as she shivers in the night air. He again closes the distance between them but only to take off his coat and wrap it around her bare shoulders.
“I’m sorry.” He speaks so quietly that she only just hears his words “It should have been me.”
Her head shakes frantically as her hand comes to rest on his cheek “Stop it. It’s not your fault, Sammy.”
“But it is. He made a deal, a deal with a demon to save me. I’m not even supposed to be here, Brooke. It should be him.” A tear rolls down her cheek just as she lifts her back from him, only to slap him hard in the face.
She is shocked, he isn’t. “I’m sorry...I’m so sorry Sammy. I didn’t mean to.” She moves to him and he steps back. He deserved that, he knows it.
“It’s okay, I’m just gonna go.” He begins to walk around her and she stops him by gripping his hand.
“Please just stay, just for tonight. I don’t want to be alone.”
“Bobby I couldn’t just not call her, Okay? The world is ending. I needed her to be safe.” He stares at his father figure with sad eyes and Bobby slowly nods his head.
“I know son, a little heads up would have been nice, I only got the basics” He motions to the grocery bag in his hand. “Just, Don’t you think she would be safe as far away from here as possible Dean?”
He shakes his head and looks through the window in order to check on her “Brooke?” He shouts and he looks at Bobby before both rush inside “Brooke?” Dean yells into the kitchen as Bobby checks the living room; they meet in the hall and notice the door to the basement is open.
“Dammit” They mutter in unison as they head downstairs.
