'Course they do, as long as they had them when they died/rose. Do the kidney's still function? No idea. Their hearts apparently still have some kind of use, though it's probably more magical and symbolic than biological.

As for whether vamps expel the blood they drink, the only evidence I can recall is a short story by Stephen King, I think it was "The Night Flyer." A reporter was following what appeared to be a serial killer who killed everyone at various small airports. He figured out where the next location would be and got there just after the vamp had killed everyone. Figuring the vamp was gone, he went into the men's room and washed his face in the sink. Then he froze as he saw a stream of red urine in the mirror, and realized the vampire hadn't actually left yet. Very creepy, which is no surprise from Stephen King of course.

As for solid waste, Max Brooks in "World War Z" says that zombies expel the flesh they eat through their intestines and anuses, though they don't get any nutrition from them. That's really gross, but I can't see Buffyverse vamps as being any different. I'm willing to believe blood they ingest is just magically absorbed, but they have no need to ingest solids and none of their organs work normally anymore, so I'd say the food just builds up in the stomach until it's forced down through the intestines. Or maybe it just rots in place, which leads to "Yes, vampires fart, and it smells really awful."
