Granted, I may be biased, but generally speaking I think the majority of people, and women in particular, are so wrapped up in their own insecurity and worry about changing at the gym that they pretty much don't notice anyone else there. Of course there are always exceptions. And I totally get that that's not exactly what you're asking. My thinking is that most people wouldn't think twice, or care. However, there's always that ONE person who may, and who may say something. It's my partner's fear when going to the gym as well, and has kept him from getting a membership.
When I'm at the gym I know I'm just busy trying to keep some level of modesty, so I don't even really notice who's around me. I can't even imagine being one of those women who hangs out naked, chatting on her cellphone on the stools in the locker room! My gym is at work, and the guys used to talk about one of the executive vice presidents who used to stand a la Captain Morgan and have long cell conversations, in the buff, one foot up on a stool. AWKWARD! :-)
I don't know what the correct answer is, but wanted to say I understand and hope you find a way to make it comfortable for yourself.
