[He remembers his children and them being dragged into the Speed Force with him. His children. Jai and Iris.

Assuming he's either time travelled or travelled to another universe, he begins looking around, taking note of Claire's present but not necessarily WHO she is. He begins searching the clinic, not yet recognizing where he is as he looks for the infants. With his eyes still skimming over the room, he asks Claire]

Have you s-seen two young children--babies. Two babies. One has a little bit of black hair and the other's--the other's a redhead. A little boy and girl.

[then he looks at her for an answer and pauses... And everything begins to sink in]

...Claire? [squints at her]

Different as in... older. Not to mention scratched up]
