I don't know why this is such a hard decision. Perhaps I am just indecisive to the core.
After you get your character "married" in HM, the mc's husband -- or wife, if you're the male protagonist -- just becomes a permanent fixture in the house, only leaving to go to work, and doesn't sleep. Their dialogue changes to cheesy things like "Together forever and never to part together forever we two and don't you know I would move heaven and earth to be together forever with youuuuu" and they cook for you on your/your character's birthday.
In a few months time you or your S.O. pops out a baby and it lies in bed for a year until it becomes a child and then you stuff it with pudding and ice cream to keep it happy. Chirp!
In some of the Japanese games you can marry someone of the same sex, but not in this one.
