- Duplicate the base and set it to Screen 100%

- Go to Layer > New Adjustment Layer > Selective Color

Yellow: +27

Magenta: -37

Magenta: -43

Neutrals
Cyan: +24
Magenta: +6
Yellow: -12

- Let's make another Selective Color (Layer > New Adjustment Layer > Selective Color)
Cyan
Cyan: +70

Magenta
Cyan: -60
Magenta: -1
Yellow: +14

- Go to Layer > New Adjustment Layer > Curves

RGB: input: 144 output: 162
RED: input: 74 output: 94
GREEN: input: 155 output: 167
BLUE: input: 120 output: 121

- Go to Layer > New Adjustment Layer > Foto Filter
Warning filter. 45%.

- Add this texture
(credit to xswaniconsx). Set to Multiply 100%

- Duplicate your base and move it on top. Overlay, 14%

- Erase what you don't want of the pic (I erased what was off the ripped notebook page)
