OHP Intake appointment phone number?

I receive SSI and have been told I am eligible therethrough for the Oregon Health Plan. I've filled out the app already and I'm pretty sure I've mailed it off but I still need to schedule an intake appointment with them so they can determine for sure whether or no I qualify. I've gone to the DHS website and tried to find the right phone number but
