One thing maybe that is definitely different here in German mindset is that people naturally don't try the way "okay, if health insurance doesn't pay it, so then I try to get the money together myself". At least I don't know it that way.
This may be a relict from times past because people were used to it then "when I'm sick, they HAVE to help me!". (Also think about the two German states in this point.)
Maybe also a consequence of "if I already struggle to get together the co-payment part of my things that NEED to be done, how should I follow that unrealistic idea to even get a full treatment payed myself?".
Some things I don't know if you can just also decide it just like that to pay a whole treatment all by yourself (except for really cosmetic surgery that depends on your own values about beauty) - I mean, if the hospitals and doctors to do that accept it just like that.
The only type of insurance were the whole bureaucracy and keeping-it is totally different, that's in private health insurance.
But this is something that usually only officials have ("Beamte") or freelancers (because law offers no possibility of letting them enter regular health insurance). Or whoever has the money to afford it. So my knowledge doesn't go very far about that.
