So, Francescu does not consciously think of Omi as a brother. However, this is not because that sort of affection isn't there; it's because Francescu calling you family is not precisely a compliment. (See: his interactions with ZERO.) Omi is something Francescu's never had before -- someone to laugh with and comfort (and take comfort from) and protect and back up. He just doesn't categorize that as "little brother," because the family Gargamel is fucked up.
The reasons why the Pyramid works for Francescu are surprisingly simple. First and foremost, he loves Ken with all his heart, and Tsuzuki recently made him realize that it didn't matter how Ken felt about anyone else. (There was a whole complex of insecurities there for a while, but they pretty much faded into the background at that point.) And now that he knows Ken loves him -- and that Ken knows Ken loves him, just as importantly -- he's secure. That reciprocation was all he needed.
The other reason that the Pyramid works for Fran is the way he feels about Omi, that not-familial bond which he doesn't know how to categorize. (There was a long and irrelevant microessay on Tsuzuki here which will eventually be expanded and posted.) Without that level of trust he'd never be secure. And he cares a lot about Omi for himself, not just because of Ken. I'd almost actually say it is love, just not romantic love. (Filia rather than eros, for the classicists in the audience.)
So to summarize, Francescu has a lover who loves him back, and a little brother (who shares his boyfriend but whatever), and he's very happy with this situation.
