Okay, I got some inspiration out of the blue! I really hope other people fill this too. Warnings for references to torture/self injury.

Pre-series: Sam decides to prove himself as a hunter. He succeeds by finishing the hunt on his own, but manages to get majorly hurt on the way, and feels like a failure for screwing up. Author's choice to end in a self-loathing angst fest or have Dean comfort Sam.
Gen, season 2 era. Sam with a serious (like, really serious, and long, cancer or something) illness, at Bobby's house, and Bobby and Dean take care of him. Slow gradual recovery, frail Sam, brotherly love and awesome dad Bobby are love <3
“Back at Niveus? I watched that kid pull one civilian out after another. Must have saved 10 people. Never stopped. Never slowed down. We’re hard on him Dean. Always have been. But in the meantime... he’s been running into burning buildings since he was, what... 12?”
I wanna see Sam, 12, running into a burning building to save people. Why not add in some protective Dean and John freaking out that his youngest has run straight into a burning building?
John puts Sam through some kind of bizarre training exercise - sensory deprivation/buried alive, or performing under sleep deprivation, or after having lost a significant amount of blood, or whatever strikes your fancy.
