She gets him to bed about half an hour later and he promptly passes out.
She takes his phone and hits redial.

“Sammy?”
“No. Me again.”
“Okay, and you’re…?”
“Jess. His girlfriend.”
A low whistle. “Girlfriend, huh.”
She knows Sam’s never had a serious girlfriend before - at least, she thinks she knows that. It’s always seemed sort of unbelievable. He’s gorgeous, smart, loving. Not the kind of guy who stays single very long.
“He never mentioned a brother.”
“Well, yeah. That sounds about right.”
Sam stirs in his sleep and she runs a hand up and down his spine. “He got drunk tonight because you didn’t call.”
“Figured he wouldn’t want me to.”
“What the hell happened between you two?”
Dean hesitates. “Ask your boyfriend.”
“Dean.”
“Tell him I didn’t forget. Okay? And Dad didn’t forget. And…if he wants to see me, I’ll be in New Mexico next week. Tell him.”
She says I promise. She hangs up the phone and curls up behind Sam, presses her lips to the back of his neck and wonders what else he’s hiding.

Two years old. Baby teeth, sweaty jacket, itching car seat straps, Dean’s solemn green eyes, hushing in dark backseats, stifling whimpers with M&Ms and desperate promises.
Monsters are real, Sammy.
The world is too big, and too small, shrunk down to sticky leather and nose-smudged windows.
Don’t be scared, Sammy.

Six years old. Angry school yard, too-small shoes, crayons and glue and peanut-butter-banana sandwiches, Dean’s smile beaming across the playground as he sails out of the big kid swings.
Dude, you were flying!
The world is bright and beautiful, yellow and sunshiney and sparkling, innocent gap-toothed smiles until a shadow blocks the sun overhead.
Get in the car, Sammy.

I loved this!!! Such an original idea set in this timeline, and it makes perfect sense that Sam would have flashbacks and issues with eights after jumping into hell.
Sucks to be sick and/or in the hospital on your birthday. Luckily, Dean's there to try to cheer Sam up.
Bonus points: Sam's cheered up the second Dean gets there, but he hides that fact because he enjoys watching Dean keep trying to come up with new ways to make him feel better.

It's so stupid, that on a solo hunt taking down a really nasty pride of chupacabras, the thing that lands Sam in the hospital on his sixteenth birthday is a migraine.
Okay, so it's not just any migraine. It's a three-day, intractable migraine, complete with touch sensitivity that wouldn't let him lie in his bed, auras that left him barely able to see, and the kind of bone-splintering pain that had him curled up on the bathroom floor, wishing that his brother or his dad or hell, any-freaking-body would show up and drive him to the hospital so he wouldn't have to call an ambulance for a headache.He ended up walking the three miles to the hospital. So, needless to say, he's feeling just fantastic

Worst birthday ever. The hunt goes bad. The innocent dies. Dean gets hauled away and framed for murder. Sam gets injured. Cas arrives too late to help.
Good thing they're in a time loop and have a chance to get it right... eventually.
Bonus points: This time, Sam's the one who doesn't realize the time loop's happening and keeps getting pissed off when so much goes wrong on his birthday (little does he know how bad it's gone before).
Oh! Extra bonus points: Sam gets injured a different way every time loop!
