Set after 8x20: Charlie takes care of Sam while Dean follows up on a lead about Kevin. Sam confesses that he's scared that this will be his last birthday since the trials are making him so sick. Charlie decides to make Sam's last birthday a great one, complete with cake and a present, which turn out to be a complete set of Harry Potter books, which she then reads to Sam.
Prompt: Season 2. During their grief over the loss of their dad, and the continuous hunt searching for the demon Dean forgets all about Sam's birthday, and really Sam wants it that way. It isn't until Bobby calls asking to speak to the 'Birthday Boy' that Dean realizes his mistake.

When he was a teen, they managed to have two dimes to rub together for a decent meal, so John let him pick out a restaurant for his birthday dinner. Of course Sam gets food poisoning. (This is the very reason I can no longer eat crepes or chimichangas.)
Nothing good ever happens on Sam's birthday and he's spent most of the day waiting for something bad to happen. So when he finds himself being thrown out of a moving car, he's not even surprised. Cue some nasty road burns etc and of course tons of h/c.
